The infamous Voodoo5 6000. This absurdly long card didn't make it to retail shelves before the collapse of 3dfx, but its sheer size and bigger-than-life status made it one of the most memorable cards ever produced.

I really don't know why you call it infamous. The Voodoo5 6000 is a really cool piece of hardware and I love mine. Playing 8xFSAA definitelly rules. I still love it.
By the way - there are even longer and bigger cards.
